Only the circle coordinator or faculty advisor may submit this form.
Has the recipient applied for membership? The individual for whom funds are being requested must have applied and been accepted by the circle to be considered for a gift.
Please describe the general financial need of this student. We do not require detailed financial information. Is this student on a Pell Grant and/or receiving other need-based financial aid? Does this student have a demonstrated financial personal or family situation such as death, serious illness, loss of job or home, etc. that prevents them from paying the membership fee? (max 200 words)
Please provide the background on the applicant’s leadership and campus involvement. (max 200 words)
Please provide a brief justification for why you believe this individual should receive the Gift of Membership and how their inclusion in ODK will benefit the individual, the circle, and the Society. (max 200 words)
Is the candidate is a member of an historically underrepresented and marginalized ethnic population and that the awarding of this gift will assist in the national goals to diversify our membership.
Please designate whether you are requesting Pay Full Fee or Pay Partial Fee. If requesting a partial fee, indicate how much you are asking the fund to cover. Circles may not request more than $94.oo.
