O∆K Circle Nickname Application Form

Please enable JavaScript in your browser to complete this form.
- Step 1 of 3
If a circle wishes to use a nickname, the proposed nickname must be reviewed by the Membership Standards Subcommittee and approved for use by the Mission Committee which has the full authority to make this decision. If a circle is to be named for an individual, that individual must be an initiated member of Omicron Delta Kappa. Circles are discouraged from selecting nicknames based on political or polarizing figures. Circles are also not to use nicknames of other circles which are already presently in use. Circles that wish to adopt a Greek-letter nickname are authorized to use letters which follow the sequence of the founding of circles.
Name of Requestor *
Please select the circle requesting a nickname.
Requestor Circle Office *
Are you requesting to use the pre-approved Greek letters? Please see the list on the ODK web site at https://odk.org/Nicknames *
Please describe the relationship and importance of the nickname to the circle. If it is a color (e.g., azure) or historical or local feature (e.g., Three Rivers), please include other places where this feature may be used at the institution. If the nickname is an individual (e.g., Jane Q. Public), describe the significance this person to the history and recognition of the circle. NOTE: use of a person's name requires permission from the family or estate. A separate letter of approval is required.
Drag & Drop Files, Choose Files to Upload
Minutes of circle meeting in which a vote was taken by the circle’s current membership requesting the designation of a nickname.
Drag & Drop Files, Choose Files to Upload

Drag & Drop Files, Choose Files to Upload
Letter of endorsement of the naming request by a surviving member of the proposed individual’s family (if circle nickname is related to an individual).
