  1. Gift of Membership requests must be submitted AND approved by the national headquarters before the circle submits the Certificate Order. Any request submitted after the certificate order has been finalized will not be considered.
  2. The intended recipient must have a submitted application in MyODK before the Gift of Membership form is submitted.
  3. Only Circle Coordinators or Faculty Advisors may submit this form.
