Gift of Membership (Singer and Key Grants) Request

Please note: 

  1. Gift of Membership requests must be submitted AND approved by the national headquarters before the circle submits the Certificate Order. Any request submitted after the certificate order has been finalized will not be considered.
  2. The intended recipient must have submitted an application in MyODK before the Gift of Membership form is submitted.
  3. The intended recipient must meet all qualifications for membership, including the residency requirement – they must have completed one semester and have a GPA at the institution.
  4. Only Circle Coordinators or Faculty Advisors may submit this form.
