To improve speed and efficiency, OΔK offers a number of web forms for circles to contact the National Headquarters.
Circle Officer Agreement
All Circle Officers must sign and submit the Circle Officer Agreement, which outlines the duties and responsibilities of an officer. The agreement only needs to be signed once by a new officer, and you should sign and submit the agreement as soon as you are appointed or elected to the role.
If you are a student officer and continuing in your same position or moving up (vice president to president, etc.), you do not need to complete a new form. Email [email protected] to inform OΔK that you are continuing, and we will update your record.
If you are a circle advisor and changing roles, e.g., circle coordinator to faculty advisor, you do not need to complete a new form. Email [email protected] to inform OΔK of your new role, and we will update your record.
Circle Webpage Update
Each OΔK circle has a web presence on odk.org. Your circle may decide to use this webpage as its only web presence, or you may choose to link to another website you host independently. You may update your odk.org web presence at any time, and we encourage you to look at it at least once a year when you transition officers. Review your circle website by clicking here. You can expect your webpage to be updated within 48 business hours of submitting this form. If you have any questions about your circle’s webpage, please contact our director of communications and programs.
Circle Nickname Application Form
If a circle wishes to use a nickname, the proposed nickname must be reviewed by the Membership Standards Subcommittee and approved for use by the Mission Committee, which has the full authority to make this decision. If a circle is to be named for an individual, that individual must be an initiated member of Omicron Delta Kappa. Circles are discouraged from selecting nicknames based on political or polarizing figures. Circles are also not to use nicknames of other circles that are already in use. Circles that wish to adopt a Greek-letter nickname are authorized to use letters that follow the sequence of the founding of circles.
Refund Form
This form must be completed by a circle advisor. OΔK will not accept any refund form not submitted and signed by a circle advisor.
OΔK will refund the initiation fees for any applicant who did not attend a ceremony during the 2025-26 academic year.
The Omicron Delta Kappa refund policy requires that all certificates and pins not issued during the ceremony be returned NO LATER THAN 30 DAYS following the ceremony. We are allowing exceptions for returns as long as the certificates and pins are received before Friday, June 26, 2026.
A $10.00 printing fee is assessed for each certificate. For any pin not returned, an additional $15.00 will be charged. If the applicant paid through the OΔK portal, we will issue a check to the applicant. If the circle paid OΔK for the fees, we will issue the check to the institution. OΔK will not refund local fees once those funds are transferred to the circle.

Share a Circle Service Story
Each year, Omicron Delta Kappa recognizes the service activity of our circles and members. We encourage engagement throughout the year, but on the National Day of Service, we take time to recognize and thank our members for their service. You can share how you or your circle have already provided service to others, or your plans for the upcoming National Day of Service, by completing this form. If your project is planned for a future date, please remember to revisit this page to provide details about the completed project. Reports
Reports are a great way to inform the National Headquarters about events and programs your circle produces or attends.
Clay Grant Report and Reimbursement
Circles that have been awarded Clay Grants must submit this form to report on their program and receive the grant funds.
Program Report
Program Reports provide the National Headquarters with vital information about the circle. Program Reports are required for Signature Programs. Did your circle host a special event for Founders’ Day? Submit a Program Report. Was your circle recognized by your university or a community agency? Submit a Program Report. You do not have to wait for the Circle Annual Report to tell us good things about your circle.
Circle Annual Report
The Annual Report is required for all circles to be in good standing. This is a great place to share the exciting things that have happened on your campus over the past year, as well as to provide feedback to the Board of Trustees and National Headquarters.
